CONSULTATION FEE

To ensure dedicated, high-quality consultation time with our surgeon and to appropriately allocate clinical resources, our practice requires a surgical consultation fee.

A $200 non-refundable consultation fee is required to schedule a consultation, for new patients, with the surgeon.

The consultation fee is due at the time of booking the appointment.

The $200 consultation fee will be applied as a credit toward the total cost of the surgical procedure if surgery is scheduled and performed with our practice.

The credit is valid for 12 months from the date of the consultation, unless otherwise specified in writing.

The credit may only be applied towards a procedure/treatment and is not transferable to other patients.

Non-Refundable Policy

The consultation fee is non-refundable, including but not limited to:

•     Patient decision not to proceed with surgery

•     Determination that the patient is not a surgical candidate

•     No-shows or late cancellations (see Cancellation Policy)

What the Consultation Includes

•     Comprehensive evaluation by the surgeon

•     Review of medical history and aesthetic goals

•     Discussion of treatment options, risks, benefits, and expected outcomes

•     Preliminary surgical plan and recommendations

•     Cost estimates and next steps

MEDSPA CONSULTATIONS

For MedSpa Consultations, there is a $75 non-refundable fee.

Your appointments are very important to the entire team at Surface Medspa and Wellness. Time allocated for a consultation or injection appointment is reserved especially for you. We have implemented this policy because we value the business of our patients and the time of our staff. All of our policies are designed to benefit our current patients and our future patients.  Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time and patients on our waiting list miss the opportunity to receive services they need.
