How is Facial Feminization in Charlotte Performed?

The array of facial feminization procedures that each individual patient will need to achieve their vision varies. However, these procedures may include:

Cheek Feminization

To achieve a more feminine facial appearance, some patients may need to alter the cheek structure so that it’s softer and creates a naturally feminine, heart-shaped contour. To achieve this may require adding fullness or reshaping the facial structure to address any of the harsher aspects of the male face. Every cheek feminization procedure is customized for the unique facial structure of the patient and may involve the use of cheek implants or facial fat grafting.

Chin Reduction

The female face has a smaller, more refined chin structure. For a more delicate facial appearance, chin reduction surgery may be one aspect of your journey towards a more feminine facial appearance. This procedure can reduce chin projection and size, create a rounder and softer look, and lengthen the vertical dimension. A range of shapes can be created based on what will achieve the most feminine appearance for your unique facial structure.

Foreheadplasty

The shape and contour of the forehead is a subtle gender signal — and for MTF patients, it’s often important to create a more feminine, smoother brow with a customized surgical procedure. The male face typically has a more prominent brow bone than the female face, and this bone often projects outwards. The boney ridge above the eyes can be reduced to significantly soften the facial structure. Every patient is unique, and Dr. Beck will work closely with you to help you achieve a truly feminine appearance based upon what will achieve the best-looking and most natural result.

Rhinoplasty

Nature gives the male body a larger nose in many cases, and reshaping it may be a necessary procedure in full facial feminization. A nose reshaping procedure, or rhinoplasty, can make a smaller, more appealing nose structure and reduce nostril size while also addressing issues such as humps on the bridge or other aspects that appear less female. In performing this procedure, it’s vital that the entire facial structure is analyzed so the result is the perfect enhancement.

Background media

Tracheal Reduction

A tracheal reduction, or tracheal shave, is a procedure to smooth the throat area so the Adam’s apple is removed. An Adam’s apple is strictly masculine, so for most MTF patients this procedure is an important one. Having a smooth, feminine neck will help you feel confident in a fully feminized look — finally free from this particularly masculine feature.

Jaw Reduction

In facial feminization, jaw reduction may be a necessary procedure to create a softer, more refined appearance. The lower facial contour can be altered and contoured to create a natural-looking feminine look that enhances the entire face and delivers an overall more delicate appearance.

Eyelid Surgery

You may want to enhance your eyes so that they’re more open, feminine, and inviting. Eyelid surgery can resolve a range of cosmetic issues including hooded dyes, saggy upper eyelids, puffiness beneath the eyes, under-eye bags, and fine lines for a smoother, fresher look that reflects your true nature in the best possible way.

Fat Grafting

Fat grafting can plump up various areas of the face with your own fat for a very natural-looking adjustment. This can be done in conjunction with facial implants or as a standalone procedure. Nothing is softer and more organic in appearance than natural fat used to plump cheeks or other parts of the face.

Facelift

For MTF facial feminization, some patients will require a custom facelift to smooth and firm the facial structure, remove excess hanging skin and tissue, and leave the face looking younger, fresher, and exceptionally feminine in appearance.

TG Hair Restoration

The transition from MTF may include resolving thinning or balding areas. Our approach to this vital aspect of feminization is to use a revolutionary system called NeoGraft®. This system allows Dr. Beck to harvest single hair follicles from the back of the head and transplant them into the thinning area, where they can flourish and fill in missing hair. The procedure is far easier to tolerate than traditional hair transplant procedures, where a strip of your scalp is removed from which to extract follicles. The tiny injury created by this individual extraction method heals quickly, and there are no scalpels, sutures, or scars.

What is the minimum age requirement for gender confirmation surgery?

Patients must be 18 years or older for gender confirmation surgery.

Can gender confirmation surgery be reversed later on?

Gender confirmation surgery involves irreversible changes to the body. While it is technically possible to attempt to reverse some aspects, such as removing breast implants or reversing a vaginoplasty, these procedures can be complex, costly, and may not always be successful.

Can facial feminization surgery be performed in stages?

Many patients choose to have FFS procedures done over a period of time rather than all at once, due to the complexity and cost of the procedures. By breaking up the procedures into stages, patients can also allow for proper healing and recovery time in between surgeries.

How much does facial feminization cost?

The cost of facial feminizationat starts at $15,000 with Dr. Beck. Call the office and schedule a personal consultation to get a more detailed quote just for you.

What is the recovery timeline?

Recovery varies based on the specific combination of procedures performed, but most patients follow a predictable progression. You will spend the first night or two resting in our comfortable surgical center as initial swelling and bruising peak over the first few days. By weeks two to three, your sutures are removed, visible bruising fades significantly, and most patients feel comfortable returning to desk work and light social activities. Over the next several months, residual swelling continues to drain, exposing softer facial contours. Normal exercise can typically resume at the two-month mark, with final nerve sensation and settled aesthetic results maturing fully between six and twelve months.

How are incisions hidden?

Dr. Beck prioritizes discreet, virtually invisible scarring by placing incisions within natural anatomical folds, inside the mouth, or within hair-bearing areas. For brow bone reduction or hairline lowering, incisions are placed along or just behind the natural hairline so that hair can grow directly through the scar. Procedures involving the jaw, chin, or cheek reshaping utilize hidden incisions inside the mouth along the gumline, leaving no external facial scars. When rhinoplasty is performed, incisions are hidden inside the nostrils or tucked across the narrow strip of tissue between the nostrils, which heals into a faint, unnoticeable line.

What are the risks of facial feminization surgery?

While Facial Feminization Surgery is a safe procedure when performed by an experienced, board-certified plastic surgeon, all surgical care carries potential risks. These include temporary or permanent numbness and altered sensation due to nerve stretching, post-operative swelling, bruising, and minor asymmetry during the healing phase. Less common risks include infection, bleeding, delayed wound healing, or hair thinning near hairline incisions. Dr. Beck and his surgical team actively minimize these risks by using advanced surgical techniques within our fully QUAD A–accredited surgical facility.

Am I a good candidate?

You may be an ideal candidate for Facial Feminization Surgery if you are 18 years of age or older and seek to soften masculine facial features—such as a prominent brow ridge, heavy jawline, or larger nose—to align your outer appearance with your gender identity. Good candidates are in overall sound physical health with well-managed medical conditions. Additionally, candidates should hold realistic expectations about surgical outcomes as well as the emotional and physical recovery process.

When should I expect to come back for postoperative appointments?

Postoperative care is essential to ensuring smooth healing and optimal aesthetic results. Your first check-in occurs within 24 to 48 hours for an initial post-op assessment, dressing change, and drain removal if applicable. You will return in seven to ten days for suture removal and hairline inspection, followed by a two-to-three-week appointment to evaluate swelling reduction and clear you for light daily activities. Long-term milestone visits are scheduled at one month, three months, six months, and one year to monitor scar maturation and final results.

What will be expected of me to get the best results?

Achieving your ideal outcome relies on a strong partnership between you and your surgical team. You will be expected to strictly follow all written post-op instructions, including maintaining elevated head positioning while resting to control swelling and keeping up with oral hygiene for intraoral incisions. Strenuous exercise, heavy lifting, and any nicotine products must be completely avoided, as nicotine severely impairs blood flow and wound healing. Finally, protecting all healing external incisions from direct sun exposure prevents hyperpigmentation and ensures scars fade beautifully.

What Are Dr. Beck’s Qualifications and Experience with FFS?

Dr. Joel Beck is a board-certified plastic surgeon with over 23 years of specialized experience in gender-affirming care and facial feminization surgery. Beyond his plastic surgery training, Dr. Beck completed an intensive fellowship in burn and complex reconstructive surgery, giving him a deep mastery of facial anatomy, soft tissue dynamics, and structural bone work. He is an active member of the World Professional Association for Transgender Health (WPATH) and performs all procedures in a private, fully QUAD A–accredited ambulatory surgery center designed for safety, comfort, and discretion.

Have You Had FFS Surgery Before But Are Less Than Happy With The Results?

If you underwent previous facial feminization surgery elsewhere and did not achieve the balanced, feminine appearance you envisioned, Dr. Beck specializes in Revision Facial Feminization Surgery. With his extensive dual-fellowship background in complex reconstructive surgery, he possesses the specialized technical skill required to correct scar tissue, refine bone structure, and restore facial harmony. We invite you to schedule a compassionate, judgment-free consultation to discuss how we can help fulfill your ultimate aesthetic goals.

Do I need to be on feminizing hormones prior to FFS?

While many patients pursuing Gender-Affirming Facial Surgery (GAFS) are on Hormone Replacement Therapy (HRT), it is not a strict requirement for all facial feminization procedures. Hormones alter soft tissue and fat distribution, but they do not alter underlying bone structures like the brow ridge or jaw, meaning bone reshaping can be safely performed regardless of hormone status. Dr. Beck follows WPATH standards of care and will review your overall transition timeline, medical history, and personal goals during your consultation to build a surgical plan tailored specifically to your journey.
